Why perimenopause changes the conversation.

Most women know heart disease is a risk. What most women don't know is that perimenopause is when that risk starts to shift — significantly.

Research published by the American Heart Association found that perimenopausal women were twice as likely to have an overall lower cardiovascular health score compared to women who were still having regular menstrual cycles — driven largely by higher cholesterol and blood sugar levels.

Here's why:

Estrogen plays a protective role in cardiovascular health — supporting healthy blood vessel function, cholesterol balance, and blood pressure regulation. As estrogen declines during perimenopause and menopause — that protection decreases.

  • Cholesterol shifts — LDL tends to rise and HDL tends to fall

  • Blood pressure increases — blood vessels become less flexible

  • Inflammation rises — increasing strain on the cardiovascular system

  • Blood sugar fluctuations — directly affect heart health over time

Women develop cardiovascular disease on average 7–10 years later than men — largely due to the protective effect of estrogen. When that protection declines — the risk catches up.

This is not something to fear. It is something to know — and support proactively.
